Enhanced Case Management
We provide ECM as a whole-person approach to facilitate care that addresses the clinical and non-clinical circumstances of eligible individuals. ECM is a collaborative and interdisciplinary approach to providing intensive and comprehensive care management services to populations of focus. The assigned Case Manager will provide seven core services at the point of care that are not limited to:
1. Outreach and engagement
2. Comprehensive assessment and care management plan
3. Enhanced care coordination
4. Health and wellness promotion
5. Comprehensive transitional care
6. Member and family support
7. Coordination of and referral to community and social support services
Individualized Service Plan

Life Skills Training & Development
Enhance your competencies with our SIL, where participants receive essential life skills training. This program is designed to equip individuals with practical skills crucial for personal and professional success. Key areas of focus include health and safety, transportation, education employment, community support effective communication, time management, and problem-solving techniques.
Qualified Mental Health Professional
